Clinical pathways for fragility fractures of the pelvic ring: personal experience and review of the literature

چکیده
Fragility fractures of the pelvic ring (FFP) are increasing in frequency and require challenging treatment. A new comprehensive classification considers both fracture morphology and degree of instability. The classification system also provides recommendations for type and invasiveness of treatment. In this article, a literature review of treatment alternatives is presented and compared with our own experiences. Whereas FFP Type I lesions can be treated conservatively, FFP Types III and IV require surgical treatment. For FFP Type II lessions, percutaneous fixation techniques should be considered after a trial of conservative treatment. FFP Type III lesions need open reduction and internal fixation, whereas FFP Type IV lesions require bilateral fixation. The respective advantages and limitations of dorsal (sacroiliac screw fixation, sacroplasty, bridging plate fixation, transsacral positioning bar placement, angular stable plate) and anterior (external fixation, angular stable plate fixation, retrograde transpubic screw fixation) pelvic fixations are described.

The functional outcome of surgically treated unstable pelvic ring fractures by open reduction, internal fixation
Abstract Background: Unstable Pelvic fracture, a result of high energy antero-posterior compression injury, has been managed based on internal fixation and open reduction. The mode of fixation in Unstable Pelvic fracture has, however, been a subject of controversy and some authors have proposed a need to address the issue of partial breach of the pelvic ring elements in these injuries. This st...
متن کاملClinical Results of Percutaneous Fixation of Pelvic and Acetabular Fractures: A Minimally Invasive Internal Fixation Technique
Background: The pelvic ring fractures (PRF) and acetabular fractures (AF) are among the major orthopedic injuriesassociated with high rates of morbidity and mortality. Open surgical stabilization is the standard treatment for themajority of these complications. Percutaneous minimally invasive surgical stabilization of the fractures has become anaccepted treatment method for th...
متن کاملSpinopelvic Fixation of Sacroiliac Joint Fractures and Fracture-Dislocations: A Clinical 8 Years Follow-Up Study
Background: Pelvic ring injuries and sacroiliac dislocations have significant impacts on patient’s quality of life. Several techniques have been described for posterior pelvic fixation. The current study has been designed to evaluate the spinopelvic method of fixation for sacroiliac fractures and fracture-dislocations. Methods: Between January 2006 and December 2014, 14 patients with sacroi...
متن کاملInguinal Abnormalities in Male Patients with Acetabular Fractures Treated Using an Ilioinguinal Exposure
Purpose: Surgeons performing an ilioinguinal exposure for acetabular fracture surgery need to be aware of aberrant findings such as inguinal hernias and spermatic cord lesions. The purpose of this study is to report these occurrences in a clinical series of adult males undergoing acetabular fracture fixation and a series of adult male cadavers. The secondary aim is to characterize these abnorma...
متن کاملFragility fractures of the pelvis in the elderly population
Introduction Epidemiological studies have shown that the incidence of fragility fractures of the pelvis in the elderly population is increasing due to ageing. These fractures are often classified as stable and managed predominantly by medical teams. Aggressive surgical management of the neck of femur fractures has resulted in reduced morbidity and mortality in patients. There are no evidence-ba...
